As a provider in the program, when you perform a risk assessment on a patient that is covered by the SmileMore program, you may submit an an assessment fee for reimbursement.  With SmileMore's personalized oral health scores, high-risk patients can unlock additional benefits. 

When your Altus Dental patients who are at risk for caries, periodontal disease and oral cancer are covered by the SmileMore program, they may be eligible for additional preventive benefits. To determine their risk level, perform an oral health risk assessment using the FREE web-based clinical risk assessment tool. A score will be delivered in real time that will help determine the additional benefits that your patient is eligible to receive. As a participating dentist in the SmileMore program, you may submit an assessment fee for reimbursement.

A patient that is covered by the SmileMore program goes to the dentist. The clinician performs a 
risk assessment. 

Behind the scenes, peer-reviewed and patented technology accurately 
predicts the patient’s risk for oral diseases.

Based on oral health scores, the patient can unlock additional preventive benefits
from Altus Dental.

Members scoring between 3 and 5 on a five-point scale are immediately eligible for enhanced benefits.**

Step 1

The dentist or hygienist performs a web-based clinical oral health assessment during an exam and receives a score in real time. Submit for reimbursement using caries risk assessment codes: D0601 - low risk, D0602 - moderate risk and D0603 - high risk.

Extra Benefits for your at-risk patients and increased revenue for you 

*Additional preventive benefits may be available if the patient's group has the SmileMore program and are subject to the provision of your patient’s Altus Dental policy. Only the clinical risk assessment performed by you can determine your patient’s eligibility for additional preventive benefits. In order to continue to be eligible for benefits, the patient must have an assessment (complete or partial) once every 24 months. Note: All additional benefits are subject to the patient’s annual maximum, coinsurance levels and other limitations. 1. Combination of prophylaxis/perio maintenance, scaling in the presence of gingival inflammation, not to exceed 4 in a calendar year. 2. The 2 additional fluorides that are covered for those who qualify based on their score are covered at any age.3. Sealants are covered for unrestored primary molars and for unrestored permanent bicuspids and molars. One sealant per tooth every 3 years all ages. 4. Either one nutritional counseling OR one oral hygiene instruction once every 36 months. 5. Drug/medicaments not to exceed 2 in a calendar year. These are dispensed from the dental office (not Rx) and include items like: Peridex, prescription strength fluoride. No age restrictions.
